Client requires support with accessing the community, paperwork and communication
Client has a scooter for mobilising outside
Client main diagnosis:
Depression
Fibromyalgia
EUPD (Emotionally Unstable Personality Disorder)
Asthma
Anxiety
Arthritis which affects mobility
Pets
The client has two small, friendly, dogs
Communication:
There are no communication barriers
Client enjoys:
Going out into the community, out for a coffee, going to groups, watching TV.
