4 hours a week – flexibly used.
Tasks/duties:
Social support to go out for walks, coffee shops, supper market etc.
Will need support in wheelchair when out. Does not like traveling in car due to travel sickness.
Main diagnosis:
-Stoke (left side weakness)
-Registered blind
-Diabetes-insulin control
Moving and handling:
Uses a rollator in the home to walk, and wheelchair when out.
What do you enjoy doing:
Likes to go out for a coffee
Enjoys football, lego and star wars
Has 2 dogs.
Driving license required: NO
An enhanced DBS certificate is applicable to this role.
